Autor uvádí, že označení -obtížný- je věcí toho, jak pacienta vnímáme a je ve své podstatě interpersonální. Poukazuje na důležitost komunikace mezi lékařem a pacientem, rozvádí, jak vybudovat terapeutický vztah, který povzbudí pacienta k tomu, aby zaujal odpovědný, kooperativní postoj k léčbě. Lékař se má především snažit, aby pacient cítil jistotu, spokojenost a vnímal že je považován za nezávislou, hodnotnou osobnost. Autor probírá i nejčastější chyby v komunikaci mezi lékařem a pacientem, věnuje se komunikačním dovednostem, které pomáhají vytvořit neohrožující, podporující prostředí.

Dále autor cituje mnoho rad, postupů a praktických příkladů jak rychle rozpoznat potenciálně obtížného pacienta a jak se s ním vyrovnat. Seznamuje čtenáře s normálními i nepřizpůsobivými reakcemi na nemoc, diskutuje poruchy osobnosti, které často působí obtíže v průběhu lékařské péče a cituje doporučené přístupy k pacientům trpícím těmito poruchami.
