Autor podává přehled o současných poznatcích s podáváním antidepresiv v léčbě chronické bolesti. Poukazuje na to, že není přesně znám a vysvětlen celý mechanismus účinku antidepresiv v této indikaci. Přesto není pochyb, že kromě pozitivního účinku antidepresivního a anxiolytického mají antidepresiva i přímý analgetický efekt. Pro jeho vysvětlení existuje několik možností. Autor zdůrazňuje, že antidepresiva v uvedené indikaci jsou velmi zanedbávána a podceňována. Zdůrazňuje, že antidepresiva by měla mít pevné místo ve schématu léčby dlouhodobé bolesti. Vysvětluje také limity dosud publikovaných kontrolovaných studií s danou tématikou.
