Sexuální dysfunkce při léčbě antidepresivy

Rizika sexuálních dysfunkcí (SD) indukovaných antidepresivy nejsou zatím dostatečně prozkoumána. Podle současných znalostí se zdá, že by mohla existovat skupina antidepresiv s vyšším rizikem SD, kam patří preparáty s převahou serotoninergního působení, a skupina s nižším rizikem, u které převažuje antiserotoninový efekt, dopaminergní a snad i noradrenergní působení. Do skupiny s předpokládaným vyšším rizikem SD patří tricyklická antidepresiva, IMAO, SSRI a SNRI. Skupinu s patrně nižším rizikem SD tvoří RIMA (moklobemid), SARI (nefazodon a trazodon), NaSSA (mirtazapin), NARI (reboxetin), NDRI (bupropion), SRE (tianeptin) a extrakt třezalky (Hypericum perforatum). Nefazodon, trazodon a mirtazapin jsou využívány v přídatné léčbě SD. Při sexuálních potížích po AD je možné nemocné převést na antidepresivum s nižším rizikem sexuálních poruch.

Antidepressant - indued sexual dysfunction

Risk of antidepressant-induced sexual dysfunctions (SD) is not yet sufficiently explored. According to our present knowledge, agents with mainly serotonergic effect might be associated with higher rates of sexual dysfunctions than antiserotonergic, dopaminergic, and probably noradrenergic drugs. A higher risk of sexual dysfunctions is supposed in a group of tricyclic antidepressants, IMAO, SSRI, and SNRI. In contrast, RIMA (moclobemide), SARI (nefazodone a trazodone), NaSSA (mirtazapine), NARI (reboxetine), NDRI (bupropione), SRE (tianeptine), and St. John-s wort extract (Hypericum perforatum) probably represent a low-risk subgroup. Nefazodone, trazodone and mirtazapine are used as adjunctive treatment for SD. When sexual side effects occur, it is possible to replace a problematic antidepressant with other agent with less sexual dysfunction.
