The article summarizes current recommendations for standard treatment modalities for patients with emotional dysregulation or borderline personality disorder and presents some innovative neuroscience-based treatment options. Standard treatment modalities include outpatient psychiatric treatment with pharmacotherapy, hospitalization, and psychotherapy. Pharmacotherapy should be prescribed sparingly in patients with emotional dysregulation and only as an adjunct to psychotherapy based on current symptoms, and its effectiveness should be continuously evaluated. Hospitalization is particularly appropriate for patients with emotional dysregulation in the short-term and ideally before life-threatening behavior occurs; long-term hospitalization without targeted intervention can be harmful. The only causal treatment for emotional dysregulation is psychotherapy, particularly targeted programs/approaches for the treatment of emotional dysregulation (e. g., dialectical behavior therapy, mentalization-based therapy, schema therapy, transference-focused psychotherapy, and others). The article further summarizes the basic principles of dialectical behavioral therapy and general effective principles of therapeutic work with patients with emotional dysregulation. In the concluding section, the article describes transcranial magnetic stimulation and fMRI neurofeedback methods as potentially effective adjuvant methods in the treatment of emotional dysregulation in the future.
