Multiple sclerosis (MS) is an autoimmune disease which affects twice as often females as males, with onset between the age of 20–40 years. Optic neuritis or myelitis are the most common first symptoms of MS. Diagnosis of MS is determined by new revised diagnostic criteria (McDonald et al, 2005) through dissemination of inflammatory process in space and time. Due to the highest inflammation activity at the very beginning of the disease it is very important to start immuno-modulatory treatment as soon as possible (beta interferons, glatirameracetate, intravenous immunoglobulins, azathioprin). Prion diseases in spite of their low prevalence play an important role in differential diagnosis of dementia. It diagnosis is currently based on clinical manifestation and auxiliary examinations (EEG, MRI, protein 14-3-3). Final diagnosis is neuropathological. Diagnosis of human prion diseases is in the Czech Republic coordinated by a National referential laboratory of human TSE/CJN. The paper summarizes briefly the topic of human prion diseases, presents a case report of a sporadic Creutzfeldt-Jakob disease in a 51 year old female patient.

Cognitive disorders in patients with vascular brain disease are relatively frequent. In spite of the significantly negative influence on quality of life, they remain often unrecognized and untreated. The authors discuss a brief review of ethiopathogenesis, diagnostics and treatment of vascular cognitive disorders.

Depressive disorder and alcohol abuse are very common psychiatric disorders with a reccurent (chronic) course. 25 % of patients with depression have problems with alcohol abuse. We often meet these patients in our practise. We have to solve how to treat these patients, if we should prescribe antidepressants nonabstinaint patients, or to force patients to abstinence, or if antidepressants may influence depression or decrease alcohol abuse.

The article describes psychosocial intervetions and rehabilitation programmes from the point of view od psychiatric rehabilitation. The mission and the rehabilitation principles are revealed. The essential ingredients that constitute rehabilitation program are described as well as the content of some of them and the evidence-based findings.
