Autorky předkládají kasuistiku nemocné s těľkou depresivní poruchou, která kladla nároky na výběr vhodného antidepresiva vzhledem ke komplikovanosti somatických onemocnění pacientky. V uváděném případě doąlo 11 let po operaci arteriovenózní malformace v mozku k manifestaci epileptických záchvatů včetně pozitivního elektroencefalografického nálezu. Navíc ąlo o trvale imunosuprimovanou pacientku po transplantaci ledviny s daląími komplikujícími chorobami (renální hypertenze, vředová choroba duodena). Po zváľení vąech okolností, zejména skutečnosti, ľe ąlo o epileptičku se závaľnými gastrointestinálními potíľemi a s přihlédnutím k trvalé medikaci, kterou nebylo moľno vysadit (ciclosporin, prednison, azathioprin, antihypertenziva, kyselina valproová), byla zvolena léčba mianserinem. Po vytitrování dávky byl tento preparát vysoce účinný a byl nemocnou velmi dobře snáąen.
