Depressive disorder is a very often and major psychiatric disease. Depression is possible in most patients succesfully treated using a right type of treatment. The main role in the treatment of depression play antidepressants. Dual antidepressants are in some cases more effective in comparison with other antidepressants in the acute and prophylactic phaze of treatment of depression, in the treatment of resistance depression. Dual AD have comparable tollerability. Efficacy and tolerability improve compliance which is important in the treatment of depressive disorders.

General anaesthesia is sometimes called a „controlled coma“, as indexed by four features: (1) the lack of motor response to instructions; (2) suppression of autonomic and skeletal responses to intra operative stimuli such as incisions; (3) absence of retrospective awareness of pain; and (4) postoperative amnesia for surgical procedures such as conversations among the medical staff. The article deals with the psychotherapy of sexuological patients in an outpatients practice. We deal with trauma of sexual abuse and violence, its psychological aspects and possibilities of psychotherapeutic treatment. Then we present care of adolescent clients with problems concerning sexual identity or orientation. In the last part we outline possibilities of psychotherapy in the care of patients with sexual identification disorders (transsexualism) and preference (deviation).

Epidemiological studies found that patients with schizophrenia are 3 times more likely to have substance use disorders than persons without mental illness. Substance abuse in schizophrenia increased positive symptoms, agressivity, suicidality, number of relapses and rehospitalisations, and poorer treatment compliance. In particular, cannabis, amphetamine, cocaine and alcohol has been reported to be with psychotogenic effect in schizophrenia. To date, the efficacy of the first generation antipsychotics in the treatment of substance abuse have been mixed. On the contrary, certain second generation antipsychotics seem to be particularly promising.

Stimulant abuse is common both globally and in the Czech Republic. Methamphetamine abuse represents great risk for physical and mental health. We mention differential diagnostics and treatment of acute conditions such as intoxications and psychoses. We also describe long-term treatment of stimulant dependence. It includes nonaddictive medication, counseling, cognitive-behavioral therapy, contingency management, family therapy and long-term treatment at therapeutic communities.

Disociative disorders are represented by different highly expressive symptoms, which are developped by the hypothetical mechanism of dissociation. Unbearable emotions, memories and thoughts are dissociated from the conscious part of psyche and represent the origin for different psychic and somatic symptoms. Most of the dissociative disorders are triggered by the psychic trauma in childhood or adulthood.
