Huntington´s disease (HD) is an autosomal dominantly inherited fatal neurodegenerative disease. HD symptoms comprise a variety of movements disorders, behavioral and other neuropsychiatric changes including severe dementia. The onset of HD is usually in the 4th decade. The diagnosis is confirmed by the genetical testing. The therapy of HD is purely symptomatic. Choreatic dyskinesias can be ameliorated by antipsychotics (tiapride, risperidone etc), GABAergics (clonazepame) or depletors of dopamine (tetrabenazine). Mixed epizodes are characterized by the mixture of depressive and manic symptoms. This state experience almost half of bipolar patiens. Manic symptoms in mixed states usually are dysphoric, with irritability, anxiety, or unpleasant activation. Such states can be difficult to distinguish from ultradian cycling, in which moods change so rapidly that they seem to blend into each other. These guidelines have been developed to assist practical neurologists, geriatricians, psychiatrists, and other specialists involved in the care of patients with dementia. The paper is grounded on scientifically based treatment procedures that are mainly based on double-blind, placebo-controlled studies and meta-analyses. We present the guidelines for the treatment of Alzheimer‘s disease, vascular dementia, Parkinson‘s disease with dementia, and dementia with Lewy bodies. These guidelines should result in rational and effective treatment procedures in the most common types of dementia.

Therapy of agitation in psychiatric disorders caused by psychoactive drugs

The article deals with a few selected topics from the range of the addictive disorders, in which the symptom of acute agitation occurs in any phase of abuse. We emphasize the typical characteristics of the states and we describe the basic algorithms of the treatment in reference to both the guidelines and our own experience. The most frequent psychiatric and somatic complications are mentioned.

The authors would like to refer to the significance of EEG biofeedback (neurofeedback) therapy in treating ADHD as the most frequent and best validated indication. Although the medical treatment of ADHD disorders was historically the only effective intervention in reducing the main symptoms, there are about 25 % of no responders. Therefore the need has arisen to offer these children other biological appro aches in the treatment of this serious disorder, since the disorder is not manageable only by pedagogical – psychological interventions.

Social rehabilitation of mentally ill persons in contemporary conditions

The article concerns with social rehabilitation of mentally ill persons in Czech Republic. This text emphasises the comprehensive approach to rehabilitation as an assumption for integration of medical and social interventions. The examples of good practice are submitted both in psychiatric hospitals, day-care centers and social services. The indication for rehabilitation programs are discussed. The evaluation of contemporary level of co-operation between medical and social services is proposed as well as the presentation of the providers´ collaboration.

Dependence on zolpidem

In this case report I am presenting a case of zolpidem dependent patient who was repeatedly hospitalised in psychiatric hospital. As such, there is an increase in number of zolpidem dependent patients. We have limited experience with the treatment of withdrawal syndrome and with the long term treatment of such patients. Therefore in my article I would like to draw attention towards the risk of zolpidem dependence and raise the interest upon this problem.
