Agomelatine in the treatment of depression under practice condition

Agomelatine is a syntetic analogue of melatonin, agonist of melatonergic MT1, MT2 and antagonist of serotonergic 5-HT2C recceptors. Agomelatine is antidepressant with unique pharmacological profile which it differs agomelatine from other antidepressant drugs like SSRI and SNRI in efficacy, tolerability. The article focuses on its efficacy in depression treatment, its diference from SSRI and SNRI, and its use in clinical practice, in patiens with somatic disorders.

Eating disorders are psychosomatic illnesses that significantly affect the patients' somatic condition. Collaboration with a paediatrician particularly involves diagnosing along with ruling out an organic cause as well as realimentation of cachexia that is frequently present. The role of a gynaecologist is then to examine and treat amenorrhoea that is one of the fundamental symptoms of anorexia nervosa and often occurs in bulimia nervosa as well.

Workaholism involves extreme time spent working, preoccupation with work to the elimination of other life areas, loss of control over the limits of one’s work and disenchantment with work, and negative emotional, social, and health consequences. The etiology of workaholism is not clear but may relate to persons with several characteristic, like compulsive, narcissistic or dependent traits, which are driven to work harder than that required from work contexts, and who have learned to place work as a main means of satisfaction compared to other lifestyle options. Treatment implications are discussed.

Prescription drug misuse and abuse are also common problems in primary care settings. Substances of abuse have profound effects on neurotransmitter systems involved in the pathophysiology of anxiety disorders and, with chronic use, may unmask a vulnerability or lead to organic changes that manifest as an anxiety disorder. Generalized anxiety disorder (GAD) follows a chronic period with low rates of remission and frequent relapses. Comorbid substance use disorder decreases the likelihood of recovery from GAD and increases the risk of exacerbation.
