The presented overview is focused on medication by anti-depressives during pregnancy and lactation and on pharmacotherapy of anxiety disorder and insomnia in these periods. It comprehensively maps known data from publications dealing with therapy by anti-depressives. Relatively new clinical data obtained during ambulant treatment of patients are reported together with experience gained at Prague Psychiatric Center (PCP). The article may serve as an advise material for a psychiatrist during intervention in a case of a pregnant or breast-feeding woman.

Our view of child dementia disorders has changed during the past decades. The article aims at putting formad currend findings and knowledge concerning aetiology, basis charakteristics, and treatment of the disorder represented child dementia. Doctors of those branches in which it can be useful in complex bio-psycho-social approach to patients are recommended to take interest in the patient’s intimate life, for which there are also epidemiological reasons. In premature ejaculation treatment, the need of complex approach is emphasized with the help of behavioral therapy enabled by simultaneous single dose of SSRI (the author has the most experience with intermittently given sertraline only on the day of coitus 4 hours before the intercourse itself in a 25 to 50 mg dose). The author reviews the current knowledge of changes in sleep architecture associated with use of antidepressants and with major depression in polysomnographic (PSG) studies. REM suppression does not seem to be necessary for antidepressant effectiveness of agents. The SSRIs, venlafaxine and reboxetine worsen sleep efficiency. On the contrary, tricyclic antidepressants, nefazodone, trazodone and mirtazapine show benefitial effect on a quality of sleep. Clinical consequences of sleep disruption in depressive disorder and therapeutic strategy in insomnia are discussed.

A significant minority of schizophrenic patients fail to respond to standard antipsychotic therapy. An alternative way of enhancing efficacy may be the use of augemntation strategies with other classes of drugs. Anticonvulsants are increasingly being used as adjunctive treatment in both acute and long-term management strategies for psychosis. Valproate is the most commonly prescribed adjunct to manage: mood disturbances as well as impulsivity, hostility and aggression in psychosis. Some reports note a beneficial role of anticonvulsants in the management of treatmentrefractory psychosis.
